Obviously, there is no single policy to best combat a global epidemic powered by an exquisitely addictive drug relentlessly promoted by powerful, predatory and innovative industry. Effective nicotine prevention requires a comprehensive public-health strategy that reshapes access, affordability, norms, and expectations, especially for adolescents whose brains are uniquely vulnerable to addiction. Other proven approaches include strong sustained public education and counter-marketing campaigns, prominent health warnings and product labels, smoke and vape-free public spaces, taxation that raises prices beyond adolescent reach, flavor restrictions and robust cessation support.
These measures are reflected in the World Health Organization’s Framework Convention on Tobacco Control and its MPOWER implementation package, which together provide a global blueprint for reducing tobacco use and countering industry interference.
Raising the legal sales age to 21 is a simple, low-cost, and highly effective policy. For younger teens, access to nicotine products is driven largely by social sources: friends, classmates, and peers just a few years older, while older adolescents and young adults are far more likely to purchase directly from retailers, both for their own use and to supply others. By removing legal access from the 17-20 age group, Tobacco 21 disrupts both pathways at once.
When the United States enacted Tobacco 21 nationwide in 2019, the dominant form of teen nicotine use in the U.S, vaping, fell by 60%, even with minimal enforcement. Few public health policies have produced such rapid, population-level impact at such low cost.

But, without enforcement, Tobacco 21 is a promise on paper; with enforcement, it becomes protection in practice. Tobacco Retail Licensing (TRL) is a cornerstone of effective Tobacco 21 enforcement. By requiring retailers to obtain and maintain a license to sell nicotine and tobacco products, TRL creates accountability where it belongs – with sellers, not kids. Licensing provides governments with a clear registry of tobacco retailers, generates dedicated funding for compliance checks, and enables meaningful penalties for violations, including suspension or revocation of the right to sell. Without licensing, age-of-sale laws are largely symbolic; with it, they become enforceable, sustainable, and self-funded. TRL turns Tobacco 21 from a policy aspiration into a functioning public-health system.
